The privacy of your personal information is an important part of our clinic policy.  We understand the importance of protecting your personal information and we are committed to collecting, using and disclosing your information responsibility. We also aim to be as open and transparent as possible about the way we handle your personal information.

 

This Privacy Policy outlines how our clinic collects, uses, discloses, and safeguards individually identifiable information provided by you for the purpose of availing medical care through our Services ensuring compliance with the Personal Information Protection and Electronic Documents Act (PIPEDA) and the Personal Health Information Protection Act (PHIPA).

For the purposes of this Privacy Policy: 

 

(i) "personal information" means information about an identifiable individual, which includes information that can be used on its own or with other information to identify, contact or locate a single person; and 

 

(ii) "personal health information" means information about an identifiable individual that relates to the physical or mental health of an individual, including information that consists of the health history of an individual's family; relates to the providing of healthcare to an individual, including the identification of a person as a provider of healthcare to an individual; relates to payments or eligibility for healthcare, or eligibility for coverage for healthcare, in respect of an individual; relates to the donation by an individual of any body part or bodily substance of an individual or is derived from the testing or examination of any such body part or bodily substance; an individual's health number; or that identifies an individual's substitute decision-maker. Personal health information may be found in medical records, treatment and examination notes (collectively, personal information and personal health information shall be referred to as "Information"). 

 

All staff members who come in contact with your personal information are aware of the sensitive nature of the information that you have disclosed to us.  Our team is trained in appropriate use and protection of your information. 

Acceptance of Privacy Policy 

We encourage everyone to read this Privacy Policy before providing us with any information and before using our Website. By providing us with any information or using our Website, you are signifying that you agree with all of the terms of this Privacy Policy. We reserve the right to amend this Privacy Policy at any time by posting a revised version on our Website. We recommend that you check periodically in order to review our current Privacy Policy. You can tell if this Privacy Policy has changed by checking the effective date that appears at the top of this Privacy Policy. Depending on the circumstances, we may also decide to send you a notice electronically that the Privacy Policy was changed. If you continue to use the Website after this Privacy Policy has changed or notice of the change has been given, you will be deemed to have accepted such changes.

 

How we Collect, Use and Disclose Patients’ Personal Information

 

Our Clinic understands the importance of protecting your personal information.  To help you understand how we are doing that, we have outlined how our clinic is using and disclosing your information.

 

Information Collection

We may gather the following types of information when you utilize our Services:

  1. Information provided directly to us, including contact details, username and password, payment information, personal health information, doctor reviews, and any other information you provide.

  2. Information from health care providers and other sources, particularly in the context of medical treatments, including medical records and additional data from various sources.

  3. Information provided when you chat with us through our live chat system

  4. We use cookies and other automatic data collection technologies to collect certain information so that you do not have to input your credentials every time you use our Website. 

 

Information Use

The information collected online may be used for the following purposes:

  1. Providing and enhancing services and treatment plans.

  2. Delivering safe and efficient patient care and identifying and ensuring continuous high- quality service.

  3. Assessing your health needs and advice of your treatment options.

  4. Enabling us to contact you and maintain communications with you, for the purposes of distributing health care information and booking/confirming appointments and treatments

  5. Invoicing for goods and services, processing credit card payments and collect on unpaid accounts.

  6. Fulfilling your requests for products, services, and information.

  7. Communicating with other treating healthcare providers including your pharmacist and family doctor (providing a separate signed consent for disclosure is on your file).

  8. Communicating with laboratories in cases where laboratory services are required.  

  9. Sending information about clinical services or general wellness.

  10. Analyzing usage patterns to enhance the Services.

  11. Preventing prohibited or illegal activities in accordance with our Terms of Use.

Obtaining Consent 

 

We obtain your consent before we collect, use or disclose your information. Normally, we ask for your consent in writing, but in some circumstances, we may accept your oral consent. Sometimes, your consent may be implied through your conduct with us, such as by agreeing to applicable terms of use and/or by accessing our services. Subject to our legal and contractual requirements and reasonable notice, you may refuse or withdraw your consent to certain of the identified purposes at any time by contacting our clinic. The period of reasonable notice will vary depending on the nature of the information and its purpose. Your consent will be obtained before or at the time of collecting the information. The purposes for the collection, use or disclosure of the information will be provided to you at the time of seeking consent. Once consent is obtained from you to use your Information, our clinic has your implied consent to collect or receive any supplementary information that is necessary to fulfill the same purposes. If you provide our clinic or our service providers with information of another individual, you represent and warrant that you have all necessary authority and/or have obtained all necessary consents from such person to enable us to collect, use and disclose such information for the purposes set forth in this Privacy Policy. There may be circumstances where you have provided information for an identified purpose, and we later need to use that information for a different purpose. In such circumstances we will seek your consent to use the information for the new purpose. 


Information Sharing

We are committed to maintaining your trust, and we may share information under the following circumstances:

  1. Authorized third-party vendors and service providers for specialized services.

  2. To those responsible for providing you with the services you request, including physicians, pharmacists, physiotherapists, psychologists, psychotherapists, nutritionists, chiropractors, nurses, counsellors, other regulated healthcare professionals and lab technicians. 

  3. Legal purposes, including responding to legal requests and protecting the rights, interests, and safety of individuals.

  4. Protected Health Information transfers as described in the PHIPA Notice. With your consent or at your direction.

 

Please note that there are circumstances where the use and/or disclosure of information may be justified or permitted or where our clinic may or must disclose information without your consent, in accordance with applicable law. For example, our clinic may disclose your information to a government institution that has asserted its lawful authority to obtain the information or where our clinic has reasonable grounds to believe the information could be useful in the investigation of unlawful activity, or to comply with a subpoena or warrant or an order made by a court, person or body with jurisdiction to compel the production of information, or to comply with court rules regarding the production of records and information, or to protect our clinic’s rights and property. When obliged or permitted to disclose information without consent, our clinic will not disclose more information than is required for the purpose that obligates or permits us to disclose such information.

 

Publicly shared information in third-party sites linked to our Services is not our responsibility.

 

No information from patients during medical consultations or appointment requests is used for marketing.

Retention, Storage and Security of Your Information 

 

We will retain your information to the extent necessary for the purposes for which it was collected as described in this Privacy Policy, to comply with applicable laws, and for reasonable backup, archival, audit, or other similar purposes. Your information is stored in secured locations and on servers controlled by our clinic, located either at our facilities or at the facilities of our service providers. Our clinic uses the standard safeguards applicable to our industry to protect the security of information in our possession, including physical, organizational and technological measures. These measures include: • Premises security; • Restricted access to files that contain information; • Technological safeguards like security software and firewalls to prevent hacking or unauthorized computer access; and • Internal password and security policies. However, no method of transmitting or storing information is completely secure. While our clinic takes appropriate measures to protect your information, there is always a risk that it may be lost, intercepted or misused. Our clinic makes no warranty with respect to the integrity of any electronic communications between us and you or with respect to the security of any such communications and you consent to our clinic’s exchange of electronic communications. 

 

Security

We implement measures to safeguard information in accordance with PHIPA and other information from loss, theft, misuse, and unauthorized access.

Some Services may contain links to third-party content. Privacy practices of these third parties are not covered by this Privacy Policy.

 

Limiting Data Collection and Do Not Track

Opt-out options are available for interest-based advertising, email blasts, text reminders

 

Changes to the Privacy Policy

This Privacy Policy may be updated, with revisions and the effective date posted. Regularly reviewing the Privacy Policy is recommended.
